1996 jetta jerks after drivin for awhil

Somebody please help! So i drive my jetta to work which is about 8 miles there and back and i have no issues.. But when i drive longer than that when the engine is really warmed up itll jerk like its starving for fuel in all gears.. Yesterday i was driving around more than usual running errands, still jerked but still drove it and made it home. Well after making it home, not even 10 min i hopped back in the car to run to the store and not even a 100 meters down the road it jerked a lot and shut off, loss of power and accerleration. Then it wouldnt turn on for a couple of tries. Was able to get it turned on and turn the car around but it was sluggish and week in power, was only able to move it another 20 ft till it shut off and i gave up. So i had my Girlfriend pick me up, i left the car there and went to the store for about 45 min. Came back and it started right up and made it home, not far up the road. I left it on and it stayed on... So bottom line it only jerks when the engine is warm or when it has been drivin for awhile, more than my 8 mile daily drive to work. It doesnt overheat... Ive been reading other post that it could be the MAF or the TPS. Somebody please help!!!!
