Blown wastegate gasket

My buddy has a RSX integra, he is very proud of his newly installed T3/T4 turbo with an internal wastegate, but after coming to find his wastegate gasket blows he was kind of disappointed in turbo. I want to bring his hope back to life. Can anyone tell me what could of happen to his waste gate gasket without typing a novel? Just throw some possiblity why it might of happen and what cause it to happen?
