Angry "Lighter" outlet too small for adaptors

I have a re-charge adaptor for my cell phone and the darn thing won't work in the lighter outlet. I have the same problem with my portable cooler.

Is the plug smaller than standard? Is there an adaptor that will give these gagets a good connection?
