1999 Mercury Mountaineer

I have been getting an ERROR CODE P0401 (EGR Flow Insufficient)

Does anyone know what this codes indicates and possibly how to correct it?

P0401 Egr Code

I too am getting this code. I've replaced the EGR valve, the Vacuum Solenoid, and on a suggestion of a friend the PCV valve all to no avail.

And now I'm also getting a P0125 Closed loop fuel CTRL Insufficient Coolant temp. Can anyone give me some direction on what to try next?
