02 olds silhouette rear ac part number

Hopefully someone knows this. My van wouldn't blow air through the rear vents on the ceiling. With some digging here I found that the actuator was stuck in one spot. I removed the actuator and was able the move the knob back to vents instead of blowing at the feet all the time. My questions are A) what is the technical name of this actuator, is it a mode actuator or a temp actuator, or something else? and B) what would be the part number for it, all it has on it is "EATON CHA512TOO3A 2094 12:58" printed on the sticker, but when I search for it on the net nothing comes up. Thanks a heap in advance!
