How would one go about replacing the antenna cable?

The am/fm went out on the radio in my truck. The actual radio still works though. I can listen to cassette tapes just fine and the clock works as well. I took the radio out and made sure everything was hooked up, which it was. Then I replaced the actual antenna, but it made no difference. So I believe it's either the antenna cable has gone out or the radio just burnt out somehow. So I'm wanting to replace the antenna cable to see if that's the problem.

So what I want to know is, how would I go about doing it? How exactly would I lead the cable from the inside of my fender all the way through to my cab? Or if you have any idea what the exact problem is, go ahead and tell me.
