Unhappy Silhouette Transmission Whine

I have a '99 Silhouette. About once a month I can hear a whine that increases with RPM's. The vehicle shifts very hard at each gear. It does not whine in neutral or park. This has happened about once a month for the past year. I took it into GM (because it was under warranty). They could not reproduce the symptoms and said it was fine. Now it seems to be doing it more (once every other day). It only seems to happen after the vehicle has warmed up. Any thoughts?

I also just changed the fluid, hoping this would help. It didn't. The fluid was not all that dark and there wasn't much grit in the pan.
