shifting problems

2000 dodge diesel auto trans. 352K miles.
have to get RPM's up to start moving, when slow down will automatically shift out of gear and will also downshift going 70 MPH.

I think at 352K it's time to pitch it (trans not truck). It's given you service for a long time. It's time to put it down.
