99 sable rear drum brakes problem

hello, have a strange one here, looking for some ideas. I have done many brake jobs on many cars so I kind of know how they work etc, but i am not a pro. I replaced the rear brake shoes on my sons 99 sable. After a month or so they started squeeling at times. Pulled the drums and noticed they were very loose, easy to slide off. Saw nothing wrong internaly. Put drums back on, and manually adjusted shoes with star wheel. Set them so shoes just barely drag when I rotate wheel. Brakes feel fine and work well. Few weeks later, same thing, squeeling and loose brakes again. Bleed the brakes, check hardware, adjust again. 2 more weeks and loose again. Replace self adjusters and springs, adjust again. Now they are loose once again. It's almost like the self adjusters are adjusting the shoes inward instead of outward. I do not have hardware backwards. have compared to diagrams etc. Close to taking car to a brake shop to see what I have done wrong. Thaks for any help.
Also, any idea on how the adjusters work on this car? tried backing it up and slamming brakes, pushing parking brake etc. can't find any self adjust methods for this car.

If I remember correctly the adjusters have different threads a right and a left hand thread. Could you have mixed them up and put them on the wrong side.
Its best to do one side at a time.
