coolant leak 2000 town country

Hi, as its getting dark I see a antifreeze leak on the pass. side just back of the front tire. Im guessing the smaller line of the two, for rear heat. I see a connection fitting then moving too the front is where anti appears. Where does that line go?? Hard too change ?? I think we may have had them all replaced a few years back so things have been apart before. Just hoping too fix myself this time.
