steering wheel repair/restoration?

I haven't seen this topic here before. On my 2000 windstar the steering wheel at the top where both my wife and I hold the wheel while driving is breaking down. The outer "skin" went from leather-like to polished to gummy. The softer foam within the wheel is now exposed. I'm sure it's a combination of holding that area, rings on fingers which put pressure, sunlight etc. Anyway, I'm wondering if anyone has done a repair, replacement... on this. It looks pretty bad and soon I think parts will be picking off.

I'd prefer to not use some sort of tape or half assed thing or steering wheel cover. Just would like to return it to it's origninal look.

Ideas??
