Grand Prix 2002 Blower motor

Blower motor was only work when dial was turn to five. Now the blower motor will no longer work. Took blower motor out and jumped it with 12 volt and fan ran just fine. Any help with what might be wrong.

If you have the manual HVAC system it sounds to me like its your blower motor resistor.

tblake is correct. It is the blower resistor, very common issue on Grand Prix's with the manual HVAC controls. It is located in the heater box behind the blower motor, just follow the wirig harness you unlugged to remove the motor and there it is. It's retained by three screws (one is a pain to get to, but luckily the hole in the resistor is slotted, so you only have to loosen it)
