Obdii Code 0440 , 0137

[1996 trek]

my check eng. light stays on when i get the codes it tells me that one of my o2 sensors has a slow responce and also a low voltage.
it also says that the evaporative emmisions system malfunction

replaced both o2 sensors and aso the bad converter have erased the codes anf they keep coming back
any ideas??
