98 Century Hard Starting when cold

My 98 centrury starts very hard when cold. The engine will fire but will only idle if you push the accelerator down hard. After it starts, it will idle fine, If you don't, it sputters and lopes at low idle. Idle air valve and throttle body have been cleaned. Any suggestions? Dirty mass air flow?

Does your car vibrate when idling?
