I have a 1991 camry with 268,000 miles. Recently it has been accumulating oil in the distributor cap. I have changed the o-ring on the distributor but that didn't help. It does take several thousand miles before I notice any hesitation because of the oil so it is not a very big leak. Any help would be appreciated.

It sounds like the shaft seal is worn. If you can't disassemble the distributor to access this seal, you will have to replace the distributor. Used distributors are not that expensive.
See if you can find and remove a clip that holds the shaft into the housing. If you can remove that clip, then the shaft should be able to be removed from the top and the seal should be accessible - and replaceable.

Looking at my Toyota repair manual it looks like there is a drift pin inside the coupling. If it is a shaft seal do you think the Toyota dealer will have one for it or will I have to find one that is close at a local parts store?

You'll have to try and find out. I have no idea...
Maybe someone else has taken this type of distributor apart.

You will have to replace the dist. This is a common problem with high milage toyotas. Somehow they damn things leak internally after that long of a time. Make sure you have some vasoline handy for the shaft when you try and put the new one in. It will take a few tries before you get it seated. If you look at the old dist shaft you will notice how smooth it is from the original machining of the part. The reman ones look as if they are sand blasted and they do NOT slide in as easily as the original comes out. Just be careful not to mess up the new O-ring when you are tring to put the dist back in, there is a pretty sharp lip on the head at the opening. Vasoline, vasoline and more vasoline!!! You might want to take some 300 grit sand paper and hit the shaft just a liitle bit. Make sure you get the right dist, there are two types. Federal and California emission.
