1990 Accord Cold weather problems?

I've been having trouble with getting my heat to work in 10 degrees(F) and under. When I start up the car to warm it up, it starts revving up and down after a couple of minutes. Sometimes it doesn't do this at all, but the heat only works everyonce in a while?

Yes, the coolant level is full. Thermostat maybe idk.
