Transmission Problem?

Ive got a 99 tahoe LT and Lately while driving it seems to shift fine until it gets a small load on it and seems to jerk like it cant make up its mind as to downshift or upshift. It doesn't slip, rpms to go high like its in neutral but it just jerks for a second and then goes on like normal. Also, I have a fairly steep driveway and sometimes when pulling out, Ill drop it in second, and it seems to go halfway up then it bogs down, again rpms dont go up, it just acts like im pulling something way to heavy (which this tahoe has never towed or pulled a thing)

Any opinions. If it is the tranny going out, has anyone been successful in swapping it with a 2500 tranny. It has 104,000 on it. Garage kept and babied.
