What new module was installed? Any SES light?

Since fuel appears to be good an ignition related problem is a possibility. Spark plugs and ignition wires.

Also, try cleaning the throttle body with carb or throttle body spray cleaner. Be sure to remove and clean the IAC valve spring loaded pintle and the seat where IAC mounts on the throttle body.

Other suggestion is to remove the electrical connector to the MAF sensor and see if it will idle properly.
