Jerky transmission shifting

I have a 2000 Safari with a check engine light that stays on, in addition it has started to shift hard and wants to die during initial acceleration. Would a vacuum leak cause all these issues?? thanks

A vacuum leak could.. A faulty TPS could cause this, also. Get that code checked to find out whats wrong, then post back the results...
