what would be the cause of toasted rod bearings?

these pictures are full sized to show detail. What causes this?
I know lack of lubrication but why the oil pump checks out good, there was still a full oil sump and filter.
is it possible this block is still good?

Was it the #1 that went? One of the theories I've heard is that's the end of the crank that has the most load on it. It's the first to get oiled from the oil pump. That's the wierd thing!

it was #2 that broke the rod but all the rod bearings where trashed in fact #5 was almost compleatly worn down

Usually on the 1998+ you hear of the #1 bearing going first, then the rest follow suit from the resultant loss of oil press and trash floating around. Has this engine ever been run low on oil? Was the oil pump p/up screen clogged? Some of these engines go 200Kmi, others crater before 100K. The only commonality you see is the ones that spin a bearing are oil burners.
