Broken crankshaft

What could cause the tip of the crankshaft of a 1992 B2200 pickup to break off? This motor has broken the tip of the crankshaft where the pulley is on two different occasions. Once, after 5 years of use, and the second 8 years after the crankshaft was replaced. This F2 motor only has 86,000 miles on it.
