Woodruff Key fell out

What would cause the woodruff key to fall out? I have replaced it twice and again, it has come out. I ordered both direct from Honda so it's the correct one (the dealer here uses the VIN number to be accurate). Any thoughts?

Thanks....

It goes in the end of the crank shaft to index the pulley to the shaft.

Re: Woodruff Key fell out

ya i know now. it should fall out
should be a really tight fit and teh washer to the crank pulley bolt should voer up the end so theres no way for it to slide out
