Car Stalls While at Idle

I have a problem with my 1995 Mustang Gt 5speed 5.0L. What happens is that the car will be idling fine when cold and then as soon as it gets hot it will randomly die with the check engine light on while its idling and has no load on the engine (happens many times at red lights). Just a little bit ago the light went on while I was doing 70 on the highway.

So far I've replaced the following

1. Distributer
2. Spark Plugs
3. Wires
4. Rotor
5. Fuel Filter

I've also cleaned the IAC to help this solve this problem. The other problem that I have is that as soon as I hook up the OBD Reader it says that there is a linkup problem, or the light goes out!

Does anybody have any insight to this problem?

Whenever I have an interface problem with a PCM, and a running problem at the same time, I check the grounds for the PCM, and for the sensors. Run down all the grounds and see if there are any bad grounds.

Well I fixed the problem.

The IAC was out of adjustment. All that I had to do was to adjust it with out it plugged in. Now it runs beautifully.
