E150 idle ramps up and down; dies when you hit the gas

I have a 1993 E150 with 5.0 L V8 engine. When the engine is cold, it will start up o.k. but the idle ramps up and down. If you put it in drive and touch the throttle, it dies. Once the engine warms up, everything runs fine.

Any ideas?
