Loss of Power, Transmission?

Everyone,

I own a 01 Jeep Grand Chke and lately I have been noticing a loss in acceleration. Sometime what I am at a stop light, I feel my car jerking as if the transmission gears are changing. When I step on the gas I notice there is not that much acceleration and shifting is not immediate. I know its transmission related. But does anyone know what the cause might be and if its an expensive repair.
