Question Not sure what it is..

My problem is every now and then when I start up my car it will make this kinda screach sound, only on the start up, the rest of the time it runs great. I'm thinking it's the Altinator belt or the starter. It is a 1993 Honda Accord

first check to see if ur belts need replacing. cracks along the ribs of the belt will cause this sound. if belts have been recently replaced...check if the belts are at the rite tension. if they are ok...check if your pulleys are making the sound. check idler pulley first. those have bearings in them that could dry up.
