95' Runner feels like a stall?

I have a 95 4Runner and any time I shift between gears as I let the clutch out and begin accelerating I get a drag. Feels bumpy like its stalling almost but not really. It gets worse if I have a heavy load, try to accelerate quickly, or am going up a hill.

Any help would be appreciated.

Welcome to the AF!

First thing I would check would be the fuel supply for volume and pressure (fuel filter, fuel pump, pressure regulator). Then injectors (maybe put some Chevron Techron or equivalent in a tank of fuel). Then VSV, VAF meter/engine coolant temperature sensor/HO2 sensor.
