how to hook this Groung loop up?

Here is an ebay listing of the ground loop isolator that I have. Exactly where inline do i hook this up. To the amp side or the deck side? And what are the two wires hanging off and where do they hook up to?

I have a 91 Acura Legend and need the Bose radio code I don't have the ref # I only have the serial # and my VIN # do I need to submit the ref # too?
