clicking in valve cover,engine rummbles, not in park or neutral

hi everybody, my 93 1.9 liter escort,wich has 200.000 miles on it.just started to make a clicking noise in the valve cover,and starts a little roughfer than usual.and when i put it in all gears(except park and neutral)the engine starts to rummble pretty bad.the clicking noise is consistent in any gear (i took of the valve cover and rocker arms and and there seems to be no problem,all fluids are correct also,timming belt appears fine but not sure)if any of you could tell me what could couse these problems,and how to fix ill greatly appreciate it thankyou...
