Dies at high rpm's please help

I have a 93 accord lx with an AT and when I floor it the rpm's are fine untill around 4500 then the tach shoots all over the place and the car dies. Then I pull over shut the car off and back on around 2-3 min latter the starts and runs fine. But if I get it back up in rpm's the same thing happens. Now it is starting to do it at lower rpm's. When it is just at an idle it will rev around 500 high then drop back to normal. It will do this over and over again. By the way this all happens when it is already warmed up. Any imput would help. for the rough idle try check the vacuum hoses. Not sure but it is possible that could also solve the high RPM problem. Usually at high RPMs ur coil would be at fault since stock coils have a hard time keeping up with the high revs esp when it gets old and worn.
