Shifter - out of gear?

I have a 92 accord automatic.... Every once in a while (including last weekend) but only a hand full of times since Ive had the car, the thing seems to slip out of gear.... hmmmm..... let me explain.

Never while driving but lets say I pull into my driveway or parking spot at work and put it in park (always after coming to a complete stop, no hill, etc.). The red square light around my "P" comes on on the dash but the Green square light around my "D4" is still on and flashing. I can't turn the car off until I fix it so I put it in reverse or neutral until I get it to park without the D4 on. Only happened to me like 5 or 6 times in 4 years. Never been a huge problem, just weird. Is it possible that the previous owner threw the thing into gear without stopping completely... I never do that.... Is it something else causing it... Is it a big deal... WTH?!?!
