Tranny... Clutch? --> Problem

Well, I've just got all sorts of problems... Well, don't know if this is usual or what...

Problems:
First off... It is usually... Unless on a hill, hard to shift into 1 and 3 usually... And when I try going 2nd to 1st... From a stop that is just to throw it into first... It's even harder to throw into gear.

Occasionally, I will have the clutch fully pressed down, and I will try to shift into 1st gear, and the gears will grind, goes along with the last problem I listed... It will be hard to throw it in gear, so I have to force it and this occassionally happens...

Heard a few things... The reason it is occasionally hard to shift into gear is because of wires to gearbox? And then people said I also have to do a clutch adjustment... Never done one... So, is it easY? What is the process to do so?
