Knocking and gas pedal vibration

I own a saturn sl 1996 1.9L DOHC Motor and standard trans. It has 162k and lately it knocks and hesitate at 55 mph - 65 mph when i go from 4th gear to 5th gear. Any sugestions is apreciated.

the cams could be off, and/or the valves could be worn
