Question How much dose it cost to change out a throw out bearing for my clutch? HELP

My throwout bearing is BAD. Any one know how much work is involved in changing it out.How much will this cost me?

What are some signs of it going bad? Slipping every once in a while? My clutch is pretty new. . . and I don't push it much. But I've noticed a small amount of slippage lately. Not all the time though. Its weird.
