99 caravan acts like it's running out of gas when warm

Trying to help my uncle out with his caravan. It's a 1999 with a 3.0. We've already made the following repairs:

EGR
Fuel pump and filter
Fuel pressure regulator
MAP sensor
crankshaft position sensor

Symptoms:

Runs and idles perfectly(seriously like new) when cold. As soon as it reaches full operating temperature, it starts cutting out when you try to accelerate. Sometimes it will actually die going down the road. I'm not leaning toward the cat because it seems to be breathing just fine. It does seem kinda hot under hood, but gauge reads fine and has plenty of flow from the pipe. Any ideas???
