Where/How to access heater core for rear?

I have a 2009 Extended version - where/how can I access the heater core for rear of the van?

No year given, I'm gonna go with a '06, should be the same for all. It's behind the left (drivers) rear quatrer trim panel. A/C will need to be recovered, the rear HVAC module assy will need to be removed and then taken apart to access the heater core.

It's a 2008. I've edited original post accordingly in case it does make a difference.
