I had a used 2010 Hyundai i45 Elite MY11 Automatic inspected before I purchased it and the mechanics were puzzled at this finding. There was what appears to be a small puncture hole in the engine cylinder near the oil cap. They said they had never seen it before and had no clue as to whether it was a problem.
