Car making strange sound

A couple weeks ago I notice that my car has started making some noise and has become a rough drive. Whenever I rev, especially between 0-30 mph, its like a mini jackhammer, or a sputtering/rumbling sound when I drive.

Any thoughts on what it could be, and what I should do?

Does the noise follow the revving of the engine? Or the speed of the wheels turning? If it's worst between 0 and 30, try flipping it into neutral at 30 and let it coast and see if the noise goes away or slows down with the speed of the engine or speed of the car.
