05 GC SXT Power Steering Leak

So here is the problem: On the 05' CG I own the Hose that comes out of the bottom of the steering gear is leaking. The hose leaks a lot especially when the gear is fully extended in one direction or the other, but I checked the reservoir and it's full. I know there is little or no fluid getting to the pump or gear as I can feel the shuddering when steering and also the sound it makes. What can cause this? Could it be the screen filter being clogged? Is that hose the return hose or the pressure hose?
