Brake pad bent

I found my front inner pad looked like this after 70K.

I must have not gotten them back in right a few months back when I had the rotors lightly turned due to a slight warp I could feel. But the braking has been just fine, equal, no pulls. Guess I was lucky.

But I did somethiing wrong on the assembly.

When you did the work, the piston was probably retracted...you put it together and the pad, somehow, caught an edge, and didn't want to move, and when the piston came out the first time you applied the brakes, it bent the backing plate?
