help with High Long Term Fuel Trim

my rought idle after hot soak problem is coming back again, never fixed it last time, I think this is 2 or 3 years doing this.
First thing I want to do is find the source of my high long term fuel trim, I hope it is related to rought idle. I suspect a vacum leak that gets worse as engine hot soaks, a bit of a stretch, but all I got.

on a hot restart, LTFT was at 22 on bank 1, 15 bank2. After few minutes, the 22 drops to 17. I plugged all 4 vacum taps on intake, no change. I sprayed carb cleaner everywhere, no change. the fuel injectors were recently rebuilt, new orings, so i doubt a leak there, this problem existed before and after cleaning. Could leaky valve stem seals cause this? I have had the upper plenum on and off lots, orings look good. So what else could it be. the LIM gasket was changed at 80K, now at 220k, so do it again?
Am I correct a vacum leak is reason for high LTFT?
Could the vacume leak be between intake and crankcase, explaining why I cannot find leak with carb spray?
Bank 1 O2 sensor, Firewall side, changed not to long ago.

I find it a little odd that pulling & plugging the vacum line from FPR will quell the rought idle, but I cannot see a change in LTFT

Re: help with High Long Term Fuel Trim

and another thing, the LTFT goes to near zero when driving, just high at idle.
