Lightbulb Info needed ASAP, please look in...

My mate is a mechanic by trade, but on his own.
And he currently has a VW LT35 2.5td 5cyl, Oct 1998, in his workshop, but it has a problem, which he thinks could be a manufacturing fault.
The problem is that the front crankshaft pully keeps sheering the key and coming off.
Its already had an engine replacement previous, because of it happening before.

Has anyone had this happen, or know of anyone who has had this happen?

Is this a manufacturing fault, or just another problem that has been overlooked.

Please help, as there is a customer waiting its return...

If nobody can answer my question, then can anyone point me in the direction of any webpages that lists recalls and defects etc.
