Stalls when revved to over 3K rpm

Looking for most common causes for a 2003 to stall when the engine is revved up past 3K RPM. This is happening when in drive or even in park.

Just replaced the Intake Air Controller (IAC) valve and it did fix the problem. The van did not stall when I tested it.
