2001 transmission problem

2001 3.4 l 110 k

When driving down the highway at 100km rpms are about 2000.
Give it alittle gas to pass or go up a hill the rpms reach 5000 and the van does not speed up stays up that high until I let of the gas then the rpms go back to around 2000../
If I give it very little gas it will speed up if I hit the passing gear revs way high and does not speed up?

Any Advice would be great

Ryanjenna
