P0171 error

1999 protege 1.6L, 190700 miles. I got a P0171 error about 60,000 miles ago and tried a few things and fixed it by replacing the intake "trunk" hose that was giving a loss of vacuum. I got the error again this past weekend. I've cleaned the MAF sensor, but it didn't seem to dirty. I've checked for vacuum leaks around the PCV and grommet and the various hoses. I didn't test the vacuum yet. The fuel pump has about 35000 miles on it and the car has been well maintained. I run seafoam (injector cleaner) through it every 5-10 K miles and there aren't any other codes (in particular O2 sensor). The light hasn't come back on yet, but I haven't driven it much. My problem is that I can't remember which of the things I did last time fixed the problem.

Update
Cleaning the mass air meter fixed the problem for now, but how do you tell when one needs to be replaced?
