transmissions problems?

My 1996 jeep jerks when pulling away at low speeds almost like the front wheels are loosing traction but apparently this is a fault that can be reqtified by some simple technique to do with realignment?

Could be the little chain that connects to the sproket in your transmission, this would cause it.
