1997 GMC Sierra starts hard cold

I have a 1997 Sierra (350 Vortec) with 110,000 miles. It starts hard when cold. When cold it does not fire on the first starting attempt. On the second and subseqent attempts it will try and fire. On the fourth or fifth try it will catch and run fine. After warming up it will fire instantly. Performance (acceleration, etc..) is fine.
The fuel pressure is ok (measured with a gauge 54 - 58 psi), have replaced the fuel pressure regulator, cleaned the injectors, replaced the fuel pump oil pressure switch (which was faulty), replaced the distributor rotor (was corroded ), coolant temperature sensor replaced, air temperature sensor checked ok. Any suggestions?

on that truck, I do believe your pressure should be 60 - 65 psi when starting, and 55 + when running. I know that is the case with my 97 with a 305 vortec.
