placard in trunk

I'm looking for a placard in the trunk of my 89 Park Avenue, and can't find one. It's supposed to contain information about a remote. I lost mine and am trying to purchase another one. Any help would be greatly appreciated. Thanks in advance. There seem to be very knowledgable people in here with regards to Buicks. I'm so thankful to have found this site.
