Question Front end issues!

2000 chevy 1-ton dually w/36,000 miles. The truck pulled to the left when driving. The truck pulled to the right when braking. The front tires are not straight, but in a "v" (widest gap being between the front of the tires) when looking at them from the front. The steering was loose. In 4x4 mode, the steering was VERY loose. The front brakes had been inspected twice with nothing found to be wrong. A front end alignment was done & the pulling to the left is barely there & no pulling to the right when braking. The steering has tightened up a bit, But the tires are still in a "v". Though not as bad as before, you can still notice it.

Please help! What is causing this problem? Is it a major problem?
