AC/Heat fan not working

I have a 2002 Chevrolet Impala. The AC/heat fan started giving me problems by working on setting 5 only and eventually going out completely. I replaced the blower motor resistor (I had done this a few years ago with success when I had the same problem) but still no fan. It's almost like the switch itself is bad. Any ideas? And if the switch is bad, how do I replace it?

remove the blower motor and apply 12volts to it...see if it is the problem...
